Your body goes through incredible changes after giving birth, and your vaginal microbiome is no exception.
During pregnancy, hormonal changes create a vaginal environment that typically favors beneficial Lactobacillus bacteria [1], [2]. After giving birth, hormone levels change rapidly, and the vaginal microbiome can shift with them [3].
This new microbial community is a normal part of the transition to postpartum life, even though it may look very different from the microbiome you had before or during pregnancy.
Here's what the current research tells us.
During pregnancy: A protective microbiome
Throughout pregnancy, the vaginal microbiome is typically dominated by Lactobacillus. These beneficial bacteria produce lactic acid, which helps keep the vaginal pH low (more acidic) and creates an environment that makes it harder for disruptive bacteria to grow [4], [5], [6].
As pregnancy progresses, the vaginal microbiome often becomes more stable, with Lactobacillus becoming more dominant [1], [2]. Researchers group vaginal microbiomes into Community State Types (CSTs). During pregnancy, Lactobacillus-dominated CSTs, including those dominated by L. crispatus or L. gasseri, are common and considered the typical healthy state [7], [8], [9].
This pregnancy-associated microbiome helps maintain a stable, acidic vaginal environment. During a vaginal birth, these microbes are among the first microorganisms to reach and help colonize a baby's gut and other body sites, including the skin, helping shape the early microbiome.
Once the baby is born, however, the hormonal environment changes rapidly, and so does the vaginal microbiome.
After birth: A normal reset
Once your baby is born, hormone levels change dramatically. Estrogen and progesterone levels quickly drop [10], which changes the vaginal environment.
After delivery:
- Lactobacillus numbers decline [7], [9], [11]
- The vaginal pH becomes less acidic [12]
- Many different types of bacteria begin to grow [8], [13]
- Overall bacterial diversity usually increases [3]
As Lactobacillus levels decrease, many women transition to a vaginal community resembling CST IV [3]. Rather than being dominated by a single beneficial bacterium, CST IV contains a broader range of bacteria [14]. Unlike the gut microbiome, where higher diversity is often linked to good health, a healthy vaginal microbiome is usually less diverse and dominated almost exclusively by Lactobacillus.
Researchers commonly see increases in bacteria such as Gardnerella vaginalis, Prevotella, and Streptococcus during the postpartum period [3]. These bacteria can be part of a normal vaginal microbiome, although some have also been linked to bacterial vaginosis.
Why does this happen?
Researchers believe several normal postpartum changes work together to reshape the vaginal microbiome, including:
- Lower estrogen levels [10]
- Less glycogen
- Changes in vaginal pH [12]
- The body's natural inflammatory response after birth [7]
Here's why: Estrogen helps the cells lining the vagina store glycogen (a sugar), an important food source for Lactobacillus. After birth, estrogen levels drop sharply, so glycogen levels fall, too. With less fuel available, Lactobacillus struggles to grow and to produce lactic acid. As these bacteria decline, the vaginal pH becomes less acidic, allowing many other bacteria to thrive.
Breastfeeding may also play a role. While breastfeeding, estrogen levels remain lower than usual, which may help explain why the vaginal microbiome often stays in its postpartum state for longer [15].
How long does recovery take?
This is one of the biggest questions researchers are still trying to answer.
Most studies have followed women for about six weeks after birth, and nearly all have found that the vaginal microbiome remained markedly different from that of pregnancy [7], [9], [11], [12].
Only a few studies have looked further ahead. Those studies found that:
- Only about half of women regained a Lactobacillus-dominated microbiome within one year
- Many women still had a different vaginal microbiome even 12 months after giving birth
- Lactobacillus crispatus, which is strongly associated with a stable, acidic vaginal environment, recovered much more slowly than other Lactobacillus species [7], [8]
Because there are still very few long-term studies, scientists don't yet know exactly when the vaginal microbiome returns to its pre-pregnancy state, or if it does for everyone. This means there's a wide range of what may be considered normal during postpartum recovery.
Does the type of birth matter?
Although babies are exposed to different microbes depending on how they're born, moms experience the characteristic postpartum shift in the vaginal microbiome after both vaginal and C-section births [7]. Scientists are still studying whether recovery differs between these groups over the long term.
Can you help your vaginal microbiome recover faster?
There are currently no studies on how to speed recovery of the vaginal microbiome after childbirth. However, following general habits that support vaginal health may still be beneficial while your body heals.
For example, studies on bacterial vaginosis suggest that a balanced, nutrient-rich diet may support a healthy vaginal microbiome [16]. But it isn't yet known whether the same holds true during postpartum recovery. If you can, aim to eat a varied diet rich in fiber, fruits, vegetables, whole grains, and fermented foods. At the same time, remember that the postpartum period can be physically demanding, emotionally challenging, and busy. Be gentle with yourself. Recovery isn't about eating perfectly every day.
Other habits that support vaginal health include avoiding vaginal douching and scented products, wearing breathable cotton underwear, and only taking antibiotics when they're medically necessary.

Research suggests that women with a vaginal microbiome dominated by Lactobacillus, especially L. crispatus, tend to have better reproductive outcomes than women with a more diverse vaginal microbiome. Most of this evidence comes from studies of women undergoing fertility treatments, in which a Lactobacillus-dominated microbiome has been linked to higher pregnancy and live-birth rates [17], [18], [19].
That doesn't mean you need to worry if you're still in the postpartum recovery period. Scientists are still learning how long it takes for the vaginal microbiome to recover after birth and whether supporting that recovery can improve fertility.
If you're planning another pregnancy—particularly if you've experienced infertility, recurrent pregnancy loss, or recurrent vaginal infections—it may be worth discussing your vaginal health with your healthcare provider. When is the best time to test your vaginal microbiome?
If you're curious about your vaginal microbiome after having a baby, timing matters. The best time to test depends on what you want to learn.
- If you want to understand the immediate impact of birth: You can test soon after giving birth. The early postpartum period is a time of major hormonal and microbial changes, so testing during this window can show how your vaginal microbiome is responding to birth.
- If you want to understand your baseline vaginal health: It may be better to wait until your menstrual cycle has returned and you've finished breastfeeding. Estrogen levels remain lower during breastfeeding, which can affect the vaginal environment and microbiome [15]. Testing earlier may show changes that are part of normal postpartum recovery rather than your longer-term baseline.
- If you're planning another pregnancy: Testing when you're actively preparing to try to conceive may be especially useful. This can give you a picture of your vaginal microbiome as you prepare for your next pregnancy, rather than testing during a period when postpartum hormones are still influencing it.
- If you're simply curious about your vaginal microbiome: Consider waiting until at least 12–18 months after birth, particularly if you're breastfeeding. Research suggests that postpartum changes can persist for many months, so testing earlier may still reflect the normal effects of postpartum hormonal changes rather than your longer-term vaginal microbiome.
If you're experiencing symptoms such as unusual discharge, odor, itching, or discomfort, you don't necessarily need to wait to seek care. A vaginal microbiome test should not replace evaluation by a healthcare professional, especially if symptoms are new, persistent, or concerning.

Your vaginal microbiome doesn't immediately return to its pre-pregnancy state after birth. Instead, it goes through a normal transition as your hormones change and your body heals. Lactobacillus may decrease, microbial diversity may increase, and these changes can continue for months or longer.
